Rural elderly villagers in the northern districts often face difficulties in accessing medical care or miss follow-up appointments due to mobility and transportation issues. Coupled with a lack of health information and medication management knowledge, this easily leads to delayed treatment or improper medication use.

Rural elderly villagers in the northern districts often face difficulties in accessing medical care or miss follow-up appointments due to mobility and transportation issues. Coupled with a lack of health information and medication management knowledge, this easily leads to delayed treatment or improper medication use.

“Smart Hub” uses telehealth and IoT technology to simulate all steps of consultation in family medicine clinic of North District Hospital. It integrates mobile electronic stethoscopes, vital sign monitors (e.g., thermometer, blood pressure monitor, glucometers, oximeters, ECG, etc.), and mobile laboratories for blood glucose (Haemoglobin A1c) and lipid test. Data will be automatically uploaded to a cloud system and Electronic Health Record Sharing System (eHRSS) in real time via Bluetooth and 5G network transmission, allowing authorized doctors to log in for instant video consultations and diagnoses.

Additionally, the system helps pharmacists of partnering organization (The Hong Kong Pharmaceutical Care Foundation Limited) count medications, confirm delivery, and provide video talks for medication guidance and health education. A mobile app is also included, enabling the elderly and their authorized caregivers to view graphical/detailed data of health indicators, facilitating and encouraging self-management of physical health.
